- Copy of your highest degree obtained
- Copy of certification or license and registration
If you are selected for a position, please note you will need the following before being cleared for work:
- A completed health assessment form
- Effective October 7, all VIP Community Services employees must be vaccinated against COVID-19
- Clinical staff such as nurses, physicians and CASACs will also need to complete an Internal Appointment Application
- Background check
- Drug test
