Open Notes

We are excited to introduce a new feature called “OpenNotes.” It means you can read the notes your doctor writes about your visit through your Patient Portal account. Reading these notes can help you and your family be more involved in your care.  

Frequently Asked Questions

After you see your doctor or another health care professional, they write a visit note. This note contains important information from your visit and becomes part of your medical record. A visit note can include: 

  • A summary of what you told the doctor about your health, also called a history. 
  • Information about your blood pressure, weight, how your lungs sounded, etc. 
  • Test results. 
  • The doctor’s assessment or diagnosis of your medical conditions or symptoms. 
  • The treatment/care plan recommended to you and discussed with you. 
  • Next steps, like upcoming tests, follow-up appointments, or referrals. 

Patients who read their visit notes have said that they: 

  • Understand their health and medical conditions better. 
  • Remember and follow their treatment plans more easily. 
  • Take better care of themselves. 
  • Take their medications as prescribed more accurately. 
  • Spot mistakes in their health record and help fix them. 
  • Feel comfortable sharing notes with caregivers and others involved in their care. 
  • Communicate more clearly with their health care team, making their partnership stronger. 
  • Read your notes after your visit to review what you discussed with your provider, like your care plan, medication instructions, and follow-up appointments. 
  • Check your notes for accuracy. If something seems wrong, tell your provider or health center so they can fix it. 
  • Read your notes between visits to remind yourself of your treatment plan and upcoming procedures, tests, or appointments. 
  • Share your notes with family or caregivers to help manage your care and keep everyone informed. 
  • Before your next visit, read your notes to remind yourself about your last conversation with your provider. Think about what you’ve done since then and any questions you may have for your provider. 

Reading notes may not be right for everyone. You do not have to read your notes if you don’t want to. 

  • For some patients, just knowing that the notes are available if they ever want to read them or share them with someone involved in their care is enough. 
  • You don’t need to read every note in your record, but looking back at them to remember what you discussed with your provider can help you feel more in control of your health. 

This might be happening because the note may not be ready yet. After your provider writes and approves it, the note will show up in your Patient Portal account. 

If you have any questions, talk to your care team. 

Reading your notes does not change the confidential relationship you have with your care team. Only you and the care team directly involved in your care can access your notes. What changes is your ability to share your notes. It’s now easier to share your medical information with a care partner, family member, or others if you choose. You are in full control of who has access to your notes, which means you control your privacy. 

To help keep your information secure: 

  • Keep your Patient Portal login name and password private. 
  • Always log out when you are done looking at your Patient Portal account or notes.

How to View Your Provider's Notes

  1. Log in to the Patient Portal: https://mycw24.eclinicalweb.com/portal2043/jsp/100mp/login_otp.jsp  
  2. Click on the Medical Records tab. 
  3. Select Visit Summary. 
  4. Select the visit you want to see. 
  5. Click Visit Summary on the right to view your provider’s notes from that visit.

Heat Advisory

The New York City Emergency Management Department (NYCEM) urges New Yorkers to take precautions during the hot weather expected on Thursday, June 11 and Friday, June 12.

The National Weather Service has issued a Heat Advisory in effect both Thursday and Friday from 12:00PM to 8:00PM. Temperatures are expected to reach the low 90s on Thursday and Friday, with heat indices up to 100°F on Thursday, June 11, and heat indices above 90°F on Friday, June 12.

Thunderstorms are likely both Thursday and Friday evening. There is a potential for isolated to scattered thunderstorms with localized heavy downpours mainly during the evening commute on Wednesday. There is a slight chance that some thunderstorms may be severe. Thunderstorms can also bring local winds gusts between 30-40 mph.

During periods of extreme heat, New Yorkers and visitors should use air conditioning to stay cool, go to a place that has air conditioning if it is not available at home, stay hydrated by drinking water at regular intervals, and limit strenuous activity, especially during the hottest parts of the day. If you don’t have air conditioning, find a cool place to visit nearby, like a friend’s place, a mall, a museum, a coffee shop, a library, or an NYC Cooling Center.
